      目的 探讨一氧化氮合酶(NOS) 、肿瘤坏死因子 α(TNF α)在门静脉高压性结肠病发生机制中的作用。方法 成年雄性Wistar大鼠19只,随机抽取8只作为正常对照组,其余大鼠按复合因素法制作肝硬化模型,10周后测量肝静脉压力梯度(HVPG),门静脉取血测定一氧化氮(NO)含量,取大鼠降结肠全层行免疫组化染色,观测黏膜及黏膜下层,进行图像分析以测定NOS、TNF α表达量。 结果 与对照组相比,模型组NO水平、HVPG显著增高(P<0.01)。诱 导型NOS(iNOS)、TNF α在结肠黏膜下层血管内皮细胞高度表达,与对照组相比差别具有 显著性(P<0.05)。内皮型NOS(eNOS)在黏膜层及黏膜下层表达差异无显著性(P>0. 05)。结论 iNOS表达增加上调NO含量促进了门静脉高压性结肠病的发生,TNF α则是促进iNOS表达增加的重要因素。

      Objective To explore the role of nitric oxide synthase (NOS) and tumor necrosis factor alpha (TNF α)in the pathogenesis of portal hypertensive colopathy (PHC). Methods Adult male Wistar rats were randomly divided into control group(n=8) and model group(n=11). Eleven rats were used to establish the model of portal hypertension (PHT) by composite factor methods. After 10 weeks, hepatic venous pressure gradient(HPVG) and nitric oxide(NO) contents were measured. The expression of NOS and TNF α in the colonic mucosa of rats were measured by immunohistochemistry. Results Compared with control group, NO contents and HPVG in model group were higher(P<0.01). The expression of iNOS and TNF α in submucosal vascular endothelial cells in model group were significantly higher (P<0.05) than that in contro l group, but the expression of eNOS in submucosal vascular endothelial cells had no significant difference between two groups(P>0.05).Conclusion The increase of iNOS expression upregulates NO contents, wihich promote the pathogenesis of portal hypertensive colopathy. TNF α is an important factor for in cogenesis of portal hypertensive colopathy. TNF α is an important factor for in creasing iNOS expression.
